To provide an array of health care services, from the nursing perspective, to educate, treat, and assist our community to achieve optimal health. This will decrease health disparities all while improving access to care, and closing the gap to available specialized services offered in the community
Vanessa Fernandes, MSN, FNP-BC
Nurse Practitioner | Advocate for Accessible Primary Care | Bilingual Healthcare Provider
Vanessa Fernandes, MSN, FNP-BC, is a dedicated Nurse Practitioner born in Boston, MA, with a deep-rooted passion for healthcare shaped by her mother’s journey from Certified Nursing Assistant to Registered Nurse. Bilingual in Haitian Creole and conversational in Spanish, Vanessa brings a culturally sensitive approach to patient care.With 15 years of experience as a Registered Nurse and 8 years as a Nurse Practitioner, Vanessa’s educational journey began with an Associate Degree in Nursing from Roxbury Community College, followed by a Bachelor’s degree from Chamberlain College of Nursing. After seven years of practice, she returned to school, earning her Master of Science in Nursing with a specialization in Family Medicine from Simmons College.
Vanessa’s diverse clinical background spans inpatient and outpatient settings, including five years at Boston Medical Center’s medical-surgical and intensive care units. There, she developed a passion for primary care, focusing on preventing avoidable hospitalizations and addressing the critical shortage of female providers in primary care—particularly among diverse women, who represent only 8% of Nurse Practitioners in the U.S. market. Determined to make a difference, Vanessa is committed to increasing representation and delivering impactful care to her patients.

Erica Jean-Pierre, RN, BSN Dedicated Dialysis and Primary Care Nurse | Future Family Nurse Practitioner
With over eight years of experience in healthcare, Erica has dedicated their career to providing compassionate, high-quality care across various settings. Their journey began in 2016 when they earned their Licensed Practical Nurse (LPN) certification from Blue Hills Vocational Regional High School. Driven by a passion for nursing, they advanced their education, obtaining an Associate of Science in Nursing (ASN) from Quincy College in 2018 and later earning their Bachelor of Science in Nursing (BSN) from Chamberlain University in 2024. Currently, they are pursuing a Master of Science in Nursing (MSN) at Chamberlain University to become a Family Nurse Practitioner.
Throughout their career, Erica has worked in long-term care facilities, home care, adult foster care assessments, LTACH units, and primary care for both adults and pediatrics. However, their true specialty lies in dialysis nursing. Their expertise in dialysis extends from direct patient care to leadership roles, including serving as the Acute Coordinator for Dialysis at Good Samaritan Hospital. Their clinical proficiency, combined with a deep commitment to patient advocacy, has made them a trusted healthcare provider in the field.
Beyond nursing, Erica is a dedicated mother who balances her career and studies with an active lifestyle. She enjoys working out five days a week, prioritizing health and wellness as an essential part of her routine. Her drive, both professionally and personally, reflects her passion for growth, resilience, and making a positive impact in the lives of others
